For Semaglutide/Tirzepitide patients
Additional information: the first two steps are very important to complete prior to you appointment.
Once the forms are submitted you will receive a lab sheet from Access labs / Labcorp please present this form to Labcorp to obtain labs prior to your consultation. Once labs are completed we will be able to complete the consultation and order you medications after the appointment. If these steps are not completed they may delay prescribing of medications.
